Lost Your Vaccine Card? Here’s What to Do

STATEWIDE — Having your card handy to show proof of vaccination could be important in certain situations.

But what if you’ve lost that little card?

First of all, don’t panic. Every time a vaccine is administered, the provider records it with the state’s immunization registry.

The Virginia Department of Health (VDH) has a way for residents to obtain a copy of their immunization record by making a request here. A Virginia Immunization Information System [VIIS] representative will contact you within 24 to 48 hours to discuss your request. You can also contact the VIIS Help Desk by calling 1-866-375-9795. The Help Desk will be staffed from 8:30 a.m. until 5:00 p.m., Monday through Friday.

You can also check your email to see if your vaccination site sent documentation, or check to see if you have a login for a patient portal where you can access this information.

But if you’re a fortunate one who hasn’t misplaced their card, then take the time to make a copy of it or take a photo of it with your phone. Do not share a copy of your vaccination card on social media where scammers can find your personal information.

Keep the original vaccination card at home in a safe place, like with your other important personal documents.
